What you will do:

- Conduct comprehensive functional and occupational assessments to evaluate patient needs.
- Develop and implement tailored treatment plans based on insightful clinical reasoning.
- Employ evidence-based interventions to enhance ADLs (Activities of Daily Living), cognitive functioning, and physical abilities among patients.
- Facilitate smooth discharge planning through effective home visits and making referrals as necessary.
- Provide insightful equipment recommendations and necessary adaptations to ensure patient safety and independence.
- Maintain thorough clinical records compliant with HCPC/NHS standards.
- Collaborate seamlessly with medical staff, nurses, physiotherapists, social workers, and discharge coordinators for optimal patient care.
- Actively participate in MDT reviews, ward rounds, and case discussions.
- Mentor and supervise junior staff, students, and support workers to foster a thriving team environment.
- Play a role in service development and audit projects, contributing to the enhancement of service delivery.
- Engage in training and educational initiatives to bolster team proficiency and knowledge.
