What You Will Do:

• Conduct strengths-based assessments to identify support needs and risks.

• Develop and review care plans that promote independence and well-being.

• Work closely with multidisciplinary professionals including OTs, mental health teams, and health trust partners.

• Apply key legislation such as the Care Act, Mental Capacity Act, and Mental Health Act to uphold safeguarding standards.

• Champion the rights, dignity, and voice of vulnerable adults and their carers.

• Maintain accurate case records and participate in safeguarding enquiries as required.
