What you will do:

  • Assess and provide treatment for a diverse caseload of adults within the community setting.
  • Develop individualised care plans tailored to meet the unique needs of each client.
  • Collaborate with a multidisciplinary team to ensure comprehensive care delivery.
  • Educate clients and their families on therapy techniques and lifestyle adaptations.
  • Engage in meetings and discussions to evaluate progress and adjust treatment approaches.


Requirements:

  • HCPC registration required.
  • A minimum of 2 years’ experience in community adult services.
  • Wheelchair experience is desirable but not essential.
  • Strong experience in community adult services.
  • Proactive and team-oriented personality.
  • Being a car driver is preferable to aid with community visits.
