What You Will Do

• Coordinate and manage effective hospital discharges for adults experiencing mental health challenges.

• Undertake assessments under the Care Act 2014, ensuring all care planning is holistic, person-centred, and outcomes-focused.

• Complete Mental Capacity Assessments and Best Interest Decisions, supporting individuals during critical transitions.

• Engage with multidisciplinary professionals across health, housing, and community services to deliver integrated and timely care.

• Lead safeguarding enquiries under Section 42 and uphold best practices in protection and risk management.

• Work within relevant legislation including the Mental Health Act, MCA/DoLS/Liberty Protection Safeguards, and local safeguarding protocols.

• Maintain accurate case recordings using Nottinghamshire’s adult social care systems and meet performance expectations.

• Provide crisis support, advocacy, and guidance for individuals and families during periods of instability or vulnerability.

What We’re Looking For

• Qualified Social Worker, registered with Social Work England.

• A minimum of 2 years’ experience in mental health services and/or acute hospital discharge settings.

• Solid knowledge of statutory responsibilities under the Care Act 2014, MCA, and Mental Health Act.

• Excellent communication, assessment, and multi-agency collaboration skills.

• Strong organisational and record-keeping capabilities with the ability to manage a fast-paced caseload.
