What you will do:

• Conduct comprehensive assessments of adults referred to the service to facilitate their independence and well-being.
• Develop and implement effective care plans in partnership with the service users, their families, and other professionals.
• Review ongoing cases ensuring that all service user needs are met and adjusting care plans as required.
• Work closely with a multidisciplinary team to deliver coordinated care.
• Provide expert advice and support to service users, offering solutions tailored to individual needs.
• Keep accurate records in line with departmental policies and procedures.

Requirements:

• A degree or equivalent qualification in social work.
• Current registration with Social Work England.
• Significant frontline experience in Social Services within the UK as a qualified Social Worker.
