This full-time role involves tackling the most challenging cases on our waiting list, conducting Care Act Assessments and Reviews, as well as performing Mental Capacity assessments. Additionally, you will be involved in support planning through our Brokerage service. Experience in working within Learning Disabilities and Autism is essential, and being familiar with Liquid Logic is a great advantage.

What you will do:

• Conduct Care Act Assessments and Reviews with consideration and professionalism.

• Carry out thorough Mental Capacity assessments to support individuals in need.

• Engage in effective support planning in cooperation with our Brokerage service.

• Focus on reducing our waiting list by addressing both annual and unscheduled reviews or implementing the first Care Act Assessment.

• Use your expertise within Learning Disabilities and Autism to ensure the highest quality of care and support.
