What you will do:

- Conduct clinical verification of prescribed medications to ensure safety, efficacy, and appropriateness.
- Provide expert clinical pharmacy advice to the multidisciplinary team (MDT).
- Counsel patients and their families or carers on all aspects of medicines use, including side effects, adherence, and discharge planning.
- Support the supply of medicines for patient leave and discharge.
- Contribute to the development of evidence-based treatment plans to support timely and successful discharge.
- Participate in ward rounds and multidisciplinary meetings as required.
- Ensure compliance with legal, professional, and local policies and procedures.

Skills and Experience Required:

- Valid registration with the General Pharmaceutical Council (GPhC).
- Minimum of 1 year post-qualification experience as a pharmacist.
- Strong clinical knowledge and decision-making skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Commitment to patient-centred care and continuous professional development.
- Experience working within mental health services is desirable.
- Familiarity with electronic prescribing systems and clinical documentation is a plus.
- A car driver is essential for this role to effectively manage the demands of different hospital sites.

Additional Information:

This post is offered on a locum basis to provide flexible support during a period of temporary staffing gaps. On-site training and support will be provided to ensure the post holder is confident and competent in their role.
