We are seeking an experienced and passionate Qualified Social Worker to join the Adults Hospital Team in Scunthorpe. Key Responsibilities

• Manage a caseload proportionate to your experience and competence, providing timely interventions.

• Conduct statutory assessments, reviews, and care planning under the Care Act 2014 and other relevant legislation.

• Develop person-centred support plans in collaboration with service users and their families or carers.

• Facilitate safe and timely hospital discharges, ensuring care packages are in place and support networks are engaged.

• Maintain detailed, accurate case notes and records in compliance with legislative and policy requirements.

• Work collaboratively with health and community professionals to provide coordinated, outcome-focused care.

• Lead or participate in meetings and case reviews to monitor progress and outcomes.

• Support safeguarding enquiries and adhere to multi-agency safeguarding procedures.

Essential Requirements

• Qualified Social Worker with Social Work England registration.

• Experience in adult social care, ideally within a hospital discharge team or hospital interface setting.

• Knowledge of key legislation including the Care Act 2014, Mental Capacity Act, and Safeguarding Adults procedures.

• Excellent communication and interpersonal skills for multidisciplinary working.

• Proficient IT skills and familiarity with electronic case management systems.
