Key Responsibilities

Manage a caseload proportionate to your experience and competence, providing timely interventions.

Conduct statutory assessments, reviews, and care planning under the Care Act 2014 and other relevant legislation.

Develop person-centred support plans in collaboration with service users and their families or carers.

Facilitate safe and timely hospital discharges, ensuring care packages are in place and support networks are engaged.

Maintain detailed, accurate case notes and records in compliance with legislative and policy requirements.

Work collaboratively with health and community professionals to provide coordinated, outcome-focused care.

Lead or participate in meetings and case reviews to monitor progress and outcomes.

Support safeguarding enquiries and adhere to multi-agency safeguarding procedures.
Essential Requirements

Qualified Social Worker with Social Work England registration.

Experience in adult social care, ideally within a hospital discharge team or hospital interface setting.

Knowledge of key legislation including the Care Act 2014, Mental Capacity Act, and Safeguarding Adults procedures.

Excellent communication and interpersonal skills for multidisciplinary working.

Proficient IT skills and familiarity with electronic case management systems.
