Join us as a Band 6 Community Occupational Therapist in the picturesque county of Somerset. This full-time role offers up to £26 per hour on a locum basis. You will be working for a period of three months in a dynamic Community Therapy Team. This is an ideal opportunity for an experienced Occupational Therapist to apply their skills and make a difference in the community, providing top-notch care and rehabilitation to patients across Pathway 2 and Pathway 3 settings.

What you will do:

• Provide in-reach therapy to patients situated in care homes across Somerset
• Collaborate closely with care home teams, social care, primary care, mental health, and CHC
• Facilitate rehabilitation for Pathway 2 patients and support assessment processes for Pathway 3
• Work towards discharging patients to their original residence following a Home First approach
• Conduct holistic assessments to ensure appropriate placements when returning home is not feasible
• Ensure rehabilitation and assessment plans are patient-centred and family-inclusive

To excel in this role, you must be registered with the HCPC and possess a full UK driving licence as travel across the county is required.
