What you will do:

- Collaborate with medical and nursing staff to assess patient needs and facilitate safe discharges
- Liaise with community services and family members to ensure comprehensive care plans
- Conduct assessments and develop intervention plans for patients
- Provide emotional support and counselling to patients and their families during hospitalisation
- Maintain detailed records and reports on patient progress and discharges
- Work primarily from the office at Barnet General Hospital, ensuring smooth transitions for residents of Enfield
