What you will do:

• Manage a defined caseload of children and families, delivering the Healthy Child Programme for the 0–5 years age group, ensuring comprehensive support is provided.
• Conduct developmental reviews and health needs assessments to identify potential risks early and provide appropriate interventions.
• Promote health and wellbeing by offering sound advice on feeding, sleep, safety, and parenting strategies to new families.
• Safeguard children by identifying concerns and liaising with social care and other professionals to ensure a robust support system.
• Encourage immunisation uptake and direct families to specialist services when necessary, ensuring the best outcomes for each child.
• Maintain meticulous records and contribute to service audits and evaluations, ensuring continuous improvement in service delivery.
