Job Purpose:
As a Qualified Social Worker, you will play a crucial role in the operational delivery of an effective Adult Social Care Service. You'll be part of an integrated team dedicated to safeguarding, support planning, and promoting the wellbeing of adults with care and support needs. You'll assess individual needs, plan and co-design support plans, regularly monitor and review these plans, manage your caseload, and ensure compliance with national and service procedures.
What you will do:
- Effectively manage a complex and varied caseload with appropriate support and guidance.
- Work within legislative frameworks, preparing reports, statements and representations to judiciary at Court hearings and applications to the Court of Protection.
- Make critical decisions and manage risks in relation to caseloads, using professional judgement and evidence-based tools.
- Communicate skilfully in complex or high-risk situations.
- Attend and represent the department at multi-disciplinary forums and perform the role of Chair when required.
- Conduct direct work with service users, carers and families in line with interagency safeguarding adults’ policy and procedures.
- Act as a care navigator for individuals with eligible social care needs in collaboration with health and other agencies.
- Plan responses to individual needs in consultation with carers and advocates, ensuring services to meet assessed needs are delivered within financial resources.
Qualification:
- Dip SW, BA or MA Social Work or equivalent / or due to receive qualification within next 5 months following application.
- Social Work England registered or due to receive registration within the next 5 months following application.
