What you will do:

• Work collaboratively within the Islington Learning Disability Partnership - Adults and Transitions, addressing both new and ongoing caseloads.
• Complete backlogs of reviews and assessments, specifically focusing on adults with learning disabilities, with efficiency and precision.
• Conduct Care Act Assessments and complete support plans tailored to young adults aged 18-25 who meet Care Act eligibility.
• Undertake case management and offer oversight for a caseload of 20-25, ensuring that all statutory work, including safeguarding, is completed.
• Be part of a dynamic team with an in-office requirement of around two days a week, ensuring personal interaction during visits and most meetings.
