What you will do:
- Manage a varied and complex caseload with professional accountability and guidance from the Lead Practitioner.
- Conduct needs assessments in line with the Care Act 2014 to identify care requirements, using strengths-based approaches to encourage independence.
- Work with safeguarding colleagues to conduct and contribute to safeguarding enquiries, ensuring outcomes align with the individual's wishes.
- Deputise in the absence of the Lead Practitioner at multi-disciplinary meetings.
- Supervise and mentor up to four non-registered social care practitioners, and guide Social Work students and newly qualified social workers.
- Provide information, advice, and collaborate with partners to address broader health care or housing needs.
- Develop cost-effective care packages in collaboration with individuals, carers, and agencies, ensuring risk minimisation and quality of life enhancements.
- Assist individuals, families, and carers in making informed choices, clarifying needs, and contributing to service planning.
- Research resources and services, identifying and reporting any gaps in service provision.
- Operate within budgetary guidelines and maintain accurate case notes and records.
