What you will do:
- Provide clinical support and advice on medication use within the CMHT Services, collaborating with clinical pharmacists, nurses, and other healthcare professionals.
- Conduct medication reviews to promote the safe and appropriate use of medicines for service users.
- Oversee the handling, storage, and transportation of medicines, ensuring adherence to Trust policies and national guidelines.
- Support medicines reconciliation on admission, discharge, and during care transitions to maintain continuity.
- Assist in developing and implementing policies and procedures related to medicines management.
- Maintain accurate and current medication records, including through electronic prescribing systems.
- Identify and address potential medication errors, discrepancies, or non-adherence.
- Offer patient education aimed at enhancing adherence and understanding of prescribed medication.
- Participate in audits, quality improvement projects, and service development within the pharmacy service.
- Travel between various team bases across the Trust is necessary, making car driving an essential component of this role.
Essential Criteria:
- Registered as a Pharmacy Technician with the Health and Care Professions Council (HCPC).
- Relevant experience in a clinical pharmacy or community mental health setting.
- Comprehensive knowledge of medication use, storage, and administration standards.
- Strong communication and interpersonal skills with a collaborative approach.
- Ability to work independently, manage priorities, and adapt to a dynamic work environment.
- Full UK driving licence with access to a car for work purposes.
Desirable Criteria:
- Experience in community or mental health settings.
- Involvement in service development or quality improvement initiatives.
- Familiarity with electronic prescribing and medicines administration systems.
