Kinship Care Assess and support kinship carers, conducting regular reviews and advocacy. Work collaboratively with children’s social workers to promote the best outcomes for children in kinship care. After Care (Continuing Care & Throughcare) Support young people transitioning from care to independence under the Children and Young People (Scotland) Act. Complete pathway plans and coordinate with education, housing, and employability services for smooth transitions. Unpaid Carers Identify and assess the needs of unpaid carers, providing access to Carer Support Plans. Strengthen local support networks in partnership with third-sector organisations. Multi-Agency Collaboration Represent the service in Child Protection Conferences, LAC Reviews, and Permanence Panels. Build strong partnerships across health, education, and voluntary sectors.
