What you will do Manage a caseload of adults receiving support from community mental health services, providing guidance and care.Undertake comprehensive mental health, risk and psychosocial assessments tailored to individual needs.Develop, implement and review personalised care plans and recovery-focused interventions to support wellbeing.Safely administer depot medication in community settings and monitor treatment effectiveness.Complete risk assessments and contribute to crisis prevention and risk management strategies.Maintain accurate and timely clinical documentation using SystemOne. Person Specification Registered Mental Health Nurse (RMN) with current registration with the Nursing and Midwifery Council (NMC).Evidence of relevant post-registration professional development and active NMC registration with no practice restrictions.Proficiency in the administration and monitoring of depot medication.Experience with SystemOne clinical system is essential.Previous experience working as a Band 6 CPN or equivalent within an Adults Community Mental Health Team.Proven experience in administering depot medication in community settings is beneficial.
