Main duties: To work in a strengths-based way with adults and their families and carers, including those who are terminally ill. This includes working with multi-disciplinary colleagues, providing advice and information, working with community supports and liaison with other agencies, seeking the help of other professionals as necessary.To have person centred conversations with people and carers in a timely way evaluating the level of risk and an evidence-based analysis of need whilst promoting independence in accordance with agreed criteria and divisional policies.To assess for and provide equipment and minor adaptations to promote independence, and to monitor these as necessary, seeking advice where appropriate.To maintain and take responsibility for effectively managing a personal caseload including complex cases in consultation with the Team Manager.To work as a member of a multi-disciplinary team liaising with other professionals including health and the voluntary sector ensuring that there is the fullest co-operation with other team members, to provide an easily accessible service geared to meeting people’s needs.To ensure, along with other colleagues in other parts of the department, that people and their carers are involved in all decisions and advocates are recommended where appropriate.To assist in the planning of services in consultation with other professionals, to enable people to remain as independent as possible.To prepare and present reports in clear and concise and logical way and to a high standard, which contribute to multi-agency and multiple disciplinary conferences and meetings and court hearings and attend these forums.To seek advice and guidance on matters that need resolution and report shortfalls in services to line managers.To offer advice to other works in the multi-disciplinary team where a social worker has a particular level of skill or knowledge.To undertake any other duties that may be required commensurate with the general level of responsibility of the post.
