Work collaboratively with reablement support workers, social workers, healthcare professionals, and partner agencies. Apply relevant Adult Social Care legislation, including the Care Act 2014 and Mental Capacity Act 2005. Complete risk assessments and manage risk using sound professional judgement. Maintain accurate, compliant records, assessments, and reports. Manage and prioritise a dynamic caseload within a fast-paced service. Provide expert advice and guidance to service users, carers, and colleagues. Participate in supervision, appraisal, and ongoing professional development activities.
