Social Worker – Reablement Assessor | £42 per hour

Contract: Temporary, Full-time
Pay: £42 per hour

Seven Resourcing are looking for an experienced Social Worker to join a dedicated Reablement team in West Lothian. This is a full-time temporary position offering a competitive hourly rate and flexible working options to support work-life balance.

In this role, you will support individuals to regain independence following illness, injury, or hospital discharge, promoting autonomy and improving quality of life through targeted reablement interventions.

Key Responsibilities of the Social Worker – Reablement Assessor:

• Undertake comprehensive reablement and care assessments to identify individual needs
• Develop and implement person-centred reablement plans focused on independence and recovery
• Work collaboratively with service users, families, carers, and multi-disciplinary teams
• Support safe and timely hospital discharges and prevent unnecessary admissions
• Monitor and review progress, adapting plans to achieve best outcomes
• Manage a caseload effectively, ensuring accurate and up-to-date record keeping
• Contribute to safeguarding processes and risk assessments where required
• Promote strengths-based practice and independence at every stage

Applicant Requirements:

• Qualified Social Worker (BA/MA/BSc/MSc in Social Work, CQSW, or DipSW)
• Registered with the SSSC
• Minimum 2 years’ post-qualifying experience, ideally within adults or reablement services
• Strong understanding of adult social care legislation and frameworks
• Experience working within multidisciplinary teams
• Enhanced DBS on the update service (or willingness to obtain)
• Full UK driving licence and access to a vehicle
• Ability to work across West Lothian as required
