What you will do:
- Provide social work support to patients within the forensic mental health wards, addressing complex care needs and facilitating holistic recovery plans.
- Collaborate with a multidisciplinary team to undertake assessments, create care plans, and deliver targeted interventions.
- Engage with patients, families, and carers to ensure the best outcomes and provide support through the mental health journey.
- Liaise with external agencies, advocating for patient’s needs and ensuring seamless care and support networks beyond the hospital setting.
- Document and report on patient progress while ensuring that all actions comply with relevant legislation and best practice standards.
