What you will do:

Perform assessments and interventions for individuals with learning disabilities, ensuring compliance with legal frameworks such as the Mental Capacity Act, the Human Rights Act, and the Care Act 2014.
Manage a case load of 15 to 20 cases, predominantly involving safeguarding and care management tasks.
Perform Community Deprivation of Liberty Safeguards (CDoLS) applications and renewals.
Engage in Court of Protection work and prepare legal documentation related to court proceedings.
Participate in duty days once or twice a month and collaborate across community settings, supported living, and specialist care homes.
Actively participate in the Continuing Healthcare and DST process, supporting individuals requiring social work intervention.
