As part of the Hospital Team, you will collaborate across different community teams to provide essential social care support. You will be integral in assisting with hospital discharges, offering expertise in social work, and enhancing community independence and wellbeing.

What you will do:

- Provide social work and social care support to individuals, carers, and families during the discharge process from hospital.
- Collaborate closely with community teams to ensure effective reablement and support, including the Community Response and Reablement Team.
- Deliver tailored social work services to people living in various care settings, focusing on community independence and wellbeing.
- Offer advice and occupational therapy services, ensuring that new clients receive comprehensive community support.
- Work within a 7-day operational framework from 8:00am to 8:00pm to meet the needs of hospital and community-based clients.
- Liaise with healthcare professionals, ensuring a coordinated approach to each individual's care plan.
