Social Worker – Hospital Discharge Team | £40 - £42 per hour

Contract: Temporary, Full-time
Pay: £40 – £42 per hour

Seven Resourcing are looking for an experienced Social Worker to join a Hospital Discharge Team in West Lothian. This is a full-time temporary position offering a competitive hourly rate alongside flexible working options to support work-life balance.

In this role, you will support safe and timely hospital discharges, working closely with individuals, families, and multidisciplinary professionals to ensure appropriate care and support arrangements are in place.

Key Responsibilities of the Social Worker – Hospital Discharge Team:

• Undertake comprehensive social care assessments to support safe discharge planning
• Work collaboratively with hospitals, community teams, families, and carers
• Develop and implement care and support plans following discharge
• Apply relevant legislation, including adult social care frameworks and safeguarding procedures
• Manage a caseload effectively, ensuring accurate and timely case recording
• Contribute to risk assessments and ensure appropriate safeguarding measures are in place
• Attend multi-disciplinary meetings and contribute to discharge planning discussions
• Promote independence, wellbeing, and person-centred outcomes

Applicant Requirements:

• Qualified Social Worker (BA/MA/BSc/MSc in Social Work, CQSW, or DipSW)
• Registered with the SSSC
• Minimum 2 years’ post-qualifying experience in adult social care or hospital discharge settings
• Strong knowledge of adult social care legislation and discharge processes
• Experience working within multidisciplinary teams in a health or social care environment
• Enhanced DBS on the update service (or willingness to obtain)
• Full UK driving licence and access to a vehicle
• Ability to work across West Lothian as required
