Key Responsibilities
As a Social Worker within the HAF Team, you will:

Lead interventions supporting new community entrants through information provision, advice, signposting, and community-based social work.

Promote independence and sustained wellbeing through community social work interventions, including joint working with occupational therapy where required.

Work closely with hospital-based teams to support timely and safe discharges, ensuring individuals, carers, and families receive appropriate social care support.

Collaborate with community teams to support individuals with learning disabilities and mental health needs, promoting inclusion and coordinated care.

Undertake assessments, care planning, and reviews in line with statutory duties, local policies, and best practice.

Apply strengths-based, person-centred approaches to empower individuals and families.

Maintain accurate and timely case records in accordance with legislative and organisational requirements.

Work effectively with multi-agency partners to ensure holistic and responsive service delivery.
