Key Responsibilities

As a Social Worker within the HAF Team, you will:

• Lead interventions supporting new community entrants through information provision, advice, signposting, and community-based social work.

• Promote independence and sustained wellbeing through community social work interventions, including joint working with occupational therapy where required.

• Work closely with hospital-based teams to support timely and safe discharges, ensuring individuals, carers, and families receive appropriate social care support.

• Collaborate with community teams to support individuals with learning disabilities and mental health needs, promoting inclusion and coordinated care.

• Undertake assessments, care planning, and reviews in line with statutory duties, local policies, and best practice.

• Apply strengths-based, person-centred approaches to empower individuals and families.

• Maintain accurate and timely case records in accordance with legislative and organisational requirements.

• Work effectively with multi-agency partners to ensure holistic and responsive service delivery.
