What you will do:
- Provide necessary information, advice, and reablement services to new members of the community, ensuring they receive comprehensive support.
- Work collaboratively with the Community Independence and Wellbeing Team to offer robust social work and occupational therapy for individuals in various care and support environments.
- Collaborate with the Hospital Team to facilitate seamless support from 8:00am to 8:00pm, seven days a week, ensuring smooth transitions during hospital discharges.
- Develop personalised care plans in partnership with individuals, carers, and families, adapting to their specific needs and enhancing their wellbeing.
- Support individuals with learning disabilities and mental health needs by providing tailored social work and community support.
