Key Responsibilities

Carry out comprehensive assessments for children with disabilities, identifying individual needs and required support.

Review and evaluate existing care packages to ensure they continue to meet the child and family’s evolving circumstances.

Work collaboratively with families, carers, and service providers to deliver effective and sustainable care solutions.

Present informed recommendations to resource allocation panels regarding care planning and service provision.
