Welcome to an exciting opportunity as a Social Worker with the Adults West Community & Wellbeing Team in the vibrant town of North Hykeham. This role promises variety, offering the chance to earn £38 per hour on a locum basis for a period of 3 months. You will be providing invaluable support to adults in the community and helping to enhance their wellbeing. The role covers the areas of Lincoln and Gainsborough, with core hours from 08:45 to 17:15, Monday to Friday, and includes a 1-hour lunch break. A minimum of 2 days a week in the office is required, especially during the induction period. What you will do:
- Conduct assessments of service user's needs, identifying appropriate support services and interventions to aid their wellbeing.
- Develop and implement individual care plans, ensuring they meet the specific needs and preferences of each person.
- Liaise with external agencies, families, and colleagues to coordinate support and enable a holistic approach to care.
- Monitor and review care plans regularly in collaboration with service users and other professionals.
- Provide advice and support on safeguarding issues, ensuring all practices are compliant with relevant legislation and policies.
- Maintain comprehensive records of assessments, interventions, and progress notes.
- Participate actively in team meetings and contribute to the ongoing development of services within the West Community and Wellbeing Team.
