What you will do:
- Work closely with adults aged 18 and over with physical disabilities, memory issues, or sensory impairments
- Conduct Care Act assessments, mental capacity assessments, and reviews
- Handle safeguarding enquiries in compliance with statutory requirements
- Collaborate with individuals, families, and partner agencies to build supportive networks and improve customer journeys
- Manage a varied caseload and develop effective, person-centred plans focused on achieving positive outcomes
- Utilize a strengths-based approach to identify personal and community resources, ensuring individuals' voices are heard

Requirements:
- Registration with Social Work England
- Degree or equivalent qualification in Social Work
- At least two years of relevant UK experience as a qualified Social Worker
- A valid driving licence is essential due to the nature of the role
