Monitor and review long-term CHC care plans.
Complete joint reviews and safeguard assessments in collaboration with health colleagues.
Conduct assessments following the Care Act 2014.
Participate in Decision Support Tool Meetings and Court of Protection matters.
Maintain thorough records and reports related to service user care.
Engage with service users, their families, and other professionals to ensure optimal care.
