What you will do:
- Conduct holistic occupational therapy assessments for adults presenting with new or changing care needs at the Front Door.
- Develop and implement person-centred intervention plans to support independence in activities of daily living.
- Provide advice and recommendations on equipment, adaptations, and community resources to enable individuals to live safely at home.
- Work collaboratively with social care teams and partner agencies to facilitate timely access to services.
- Contribute to safeguarding adults and ensure all assessments and interventions comply with relevant legislation and best practice standards.
- Maintain accurate and timely records and reports in line with organisational policies.
- Participate in team meetings, case discussions, and continuous professional development activities.
Requirements of the Occupational Therapist:
- Degree or equivalent in Occupational Therapy.
- Recent UK experience working as an Occupational Therapist.
- Recent NHS experience.
- Registered with the Health and Care Professions Council (HCPC).
