What you will do:

- Provide high-quality care coordination for older adults, developing and implementing recovery plans tailored to individual needs.
- Collaborate closely with multidisciplinary teams to ensure comprehensive support and effective communication between services.
- Conduct assessments to determine service users' needs and recovery goals.
- Maintain detailed records and implementation reports to track progress and adjust care plans as necessary.
- Engage with families and carers to provide necessary education and support, fostering a holistic environment for recovery.
