Role Overview:

As a Qualified Social Worker in the Adults Community Mental Health Team, you will support adults experiencing severe and enduring mental health challenges. You will play a key role in promoting recovery, independence, and wellbeing, while meeting statutory responsibilities in partnership with NHS colleagues and community agencies. This role can be offered on a locum or permanent basis, allowing flexibility to suit your career goals.

What you will do:

• Undertake assessments, reviews, and support planning under the Care Act 2014.

• Complete comprehensive social circumstances reports and holistic assessments with a focus on risk and recovery.

• Develop person-centred care plans that promote independence and community integration.

• Work within the Mental Health Act 1983 framework, including discharge planning.

• Contribute to Mental Health Tribunals, CPA reviews, and professional meetings.

• Identify and manage safeguarding concerns and complex risks in partnership with clinical colleagues.

• Work within a multidisciplinary team and liaise with a range of partner agencies.

• Manage a varied caseload, coordinate care packages, and complete carers’ assessments.

• Maintain accurate case records and actively engage in supervision and ongoing professional development.

Person Specification:

Essential Requirements:

• Degree in Social Work (BA / MA / DipSW).

• Registration with Social Work England.

• Experience in adult social care or mental health services.

• Knowledge of relevant legislation, including the Care Act 2014 and Mental Health Act 1983.

Desirable:

• Experience working within a Community Mental Health Team.

• Experience completing Mental Capacity Assessments.

• AMHP training or a strong interest in progressing towards AMHP status.
