What you will do:

• Take responsibility for a caseload of children with life-limiting or life-threatening conditions, providing holistic, family-centred nursing care.
• Promote and support the independence of children with a range of paediatric conditions within the community.
• Liaise with local statutory and voluntary agencies, offering a coordinated approach to care alongside other healthcare professionals.
• Provide training, advice, and support for parents, families, and caregivers, ensuring a comprehensive understanding of each child's needs.
• Act as the key worker for families, ensuring seamless care and support throughout the treatment process.
