What you will do:

• Conduct holistic, strengths-based assessments to understand clients’ abilities and needs.

• Develop tailored reablement plans focused on maximising independence.

• Collaborate with reablement support workers, social care colleagues, and health partners to provide cohesive care.

• Provide specialist advice on equipment recommendations and moving & handling guidance.

• Support safe and timely discharges to prevent unnecessary hospital admissions.

• Assess care needs for individuals receiving long-term double-handed care as part of a time-limited project.

Requirements:

• Registered with the Health and Care Professions Council (HCPC).

• Full UK driving licence for community travel.
