What you will do:
- Provide a high-quality, person-centred occupational therapy service within adult social care reablement.
- Complete strengths-based assessments and develop outcome-focused reablement plans.
- Recommend equipment and adaptations, and provide moving and handling advice.
- Collaborate with individuals, carers, and partner agencies for positive outcomes.
- Undertake occupational therapy assessments in various environments, identifying strengths and needs.
- Support adults to achieve or maintain skills in daily living activities.
- Use clinical reasoning to assess functional ability and recommend interventions, equipment, and adaptations.
- Contribute to the prevention of escalation in need through responsive interventions.
- Work jointly with other professionals to coordinate effective support.
- Monitor progress and adjust interventions based on changes in circumstances.
- Maintain accurate and timely case records in line with professional standards.
- Apply safeguarding principles to protect adults from harm.
Requirements:
- HCPC Registration is required.
- A valid driver's licence is essential for this role, allowing you to travel across Enfield to meet the needs of service users efficiently.
