What You Will Do
Work as a key member of a multidisciplinary team including social workers, occupational therapists, and physiotherapists to deliver integrated health and social care services.
Manage a caseload of individuals with complex needs, ensuring coordinated and person-centred care.
Provide professional guidance and support to junior occupational therapists, particularly when managing complex cases.
Coordinate care planning with a range of professionals to ensure service users receive the most appropriate support.
Maintain accurate and comprehensive case records, ensuring effective case management and service delivery.
Promote efficient and cost-effective care, making full use of resources available through Social Services, Health, the Council, and partner organisations.
Requirements
Registered Occupational Therapist with current HCPC registration.
Experience working within adult community or social care settings.
Strong assessment, care planning, and case management skills.
A valid driving licence is required.
