We offer competitive pay rates: Weekdays at £21 per hour (umbrella/LTD), nights/Saturdays at £25.29 per hour (umbrella/LTD), and Sundays/Bank Holidays at £30 per hour (umbrella/LTD). What you will do:
- Deliver rapid-response community assessments to determine the immediate needs of patients.
- Provide short-term intervention and support designed to prevent hospital admissions.
- Participate in care calls and conduct community visits across your designated areas, promoting proactive healthcare.
- Maintain accurate and timely documentation, ensuring seamless communication with the healthcare team.
- Collaborate closely within the wider UCR and community health teams to provide comprehensive patient care.
Requirements of the Nurse:
- RGN qualification with NMC registration and a valid live pin.
- Recent UK nursing experience is essential, with community/district nursing experience required.
- Prior experience in UCR is preferred; however, care assistant experience is also considered.
- Must have access to your own car for travel between community visits.
- Ability to independently manage responsibilities in a fast-paced community setting is crucial.
