What you will do:
- Conduct comprehensive assessments and formulate care plans for patients within the Adults Community Mental Health Team.
- Deliver high-quality care coordination and mental health nursing interventions.

- Provide crucial input and support to the team's front door initiatives, ensuring smooth assessment processes.

Requirements of the Community Psychiatric Nurse:
- Hold a degree or equivalent qualification in Mental Health Nursing.
- Bring significant experience as a Mental Health Nurse.
- Maintain current NMC Registration, ensuring adherence to professional standards.
- Exhibit excellent communication and interpersonal skills, fostering positive relationships with patients and team members.
