Role Overview
An excellent locum opportunity is available for an experienced Social Worker to join the Hospital Discharge Team in Greenwich. This role offers both part‑time and full‑time options, making it ideal for practitioners seeking flexibility alongside impactful frontline work.
You will play a vital role in the Discharge to Assess (D2A) process by coordinating safe, timely discharges, completing Care Act and Mental Capacity assessments, and developing strengths‑based support plans for adults moving from hospital into community settings. Working as part of a multidisciplinary team, you will ensure discharge arrangements across pathways 1, 2, and 3 are delivered safely, effectively, and in line with statutory duties.

Key Responsibilities / What You Will Do
- Facilitate safe and timely hospital discharges using the D2A model and local policy.
- Complete Care Act assessments and Mental Capacity Act (MCA) assessments, including Best Interest decisions.
- Develop personalised, strengths‑based discharge and care plans.
- Coordinate community services, short‑term reablement, or placement pathways.
- Work collaboratively with MDT colleagues, patients, carers, and families.
- Identify safeguarding concerns and act in accordance with statutory procedures.
- Maintain clear, accurate, and timely case notes while managing a busy caseload.
Requirements / Candidate Criteria
Essential
- Qualified Social Worker with Social Work England registration.
- Experience in hospital discharge, reablement, or adult social care.
- Strong knowledge of:
- Care Act 2014
- Mental Capacity Act 2005
- D2A pathways
- Safeguarding Adults procedures
- Ability to work independently and within MDT environments.
- Excellent communication, assessment, and care‑planning skills.
Desirable
- Experience within a London borough hospital discharge team.
- Familiarity with local Greenwich pathways and community resources.
