What you will do:
- Assess, plan, implement, and evaluate specialist nursing care for patients with diabetes in acute hospital settings.
- Provide expert clinical advice on managing complex diabetes cases, including Type 1, Type 2, gestational, and secondary diabetes.
- Manage patients with diabetic emergencies such as DKA, HHS, and severe hypoglycaemia.
- Review and optimise insulin therapy, employing basal-bolus regimens, insulin pumps, and CGM systems.
- Support safe prescribing and adjustment of diabetes medications in liaison with medical teams.
- Deliver specialist education to patients and families on diabetes self-management, insulin usage, and blood glucose monitoring.
- Provide advice on diabetes management during acute illness, surgery, fasting, and steroid therapy.
- Engage in multidisciplinary ward rounds with consultants and other healthcare professionals.
- Develop and execute individualised discharge plans to ensure continuous diabetes care.
- Lead and deliver training programs for nursing, medical, and healthcare staff to elevate standards of diabetes care.
- Monitor and audit diabetes care standards, driving initiatives for service improvement.
- Maintain accurate and comprehensive patient records, ensuring professional standards are met.
- Act as a clinical resource for healthcare professionals handling diabetes cases.
- Support patients using advanced diabetes technologies, such as insulin pumps and CGM systems.
- Contribute to research and clinical audits that enhance diabetes care.
- Provide emotional support and counselling to patients facing new diagnoses or complications.
