What you will do
- Conduct assessments for adults and children with disabilities, providing equipment and adaptations to promote independence and safety.
- Complete complex moving and handling risk assessments and develop effective management plans.
- Involve service users and carers in the assessment process, supporting personalised plans and facilitating access to Direct Payments.
- Work collaboratively within a multidisciplinary team to deliver high‑quality care and support.
- Regularly review patient progress and adjust care plans to ensure the best possible outcomes.
Requirements
- Must be registered with the Health and Care Professions Council (HCPC).
- A valid driver’s licence is required to carry out home visits and engage effectively within the community.
