What you will do:
- Manage a caseload that includes highly complex patients, providing specialist assessments within your identified area of expertise.
- Deliver therapy interventions in community settings, including crisis response services, rehabilitation wards, clients' homes, and care homes.
- Continuously assess, implement, and evaluate individual care programmes tailored to patients' needs.
- Collaborate with a multidisciplinary team, including consultants, GPs, nursing staff, and allied health professionals, to ensure effective patient management and successful discharges.
- Provide clinical education and training to staff within community services.
Requirements:
- HCPC registered Occupational Therapist.
- Valid UK driving licence.
- Ability to work across a variety of community settings.
- Strong assessment, rehabilitation, and multidisciplinary working skills.
