What you will do:
- Deliver high-quality physiotherapy care to service users within the Unplanned Care speciality.
- Independently assess, diagnose, and plan the care of patients, utilising your expertise to improve patient outcomes.
- Organise and manage a varied caseload, treating patients with age-related diseases and chronic medical conditions.
- Participate in a multidisciplinary team, providing specialist knowledge and skills.
- Develop, evaluate, and adjust treatment programmes, making necessary referrals to maximise rehabilitation potential.
- Plan patient discharges in collaboration with the multidisciplinary team, ensuring smooth transitions within services.
- Mentor junior staff and manage caseloads to maintain high service standards.
- Determine and provide appropriate walking aids and educate patients on their safe use.
- Serve as the physiotherapy lead in the multi-disciplinary team, integrating physiotherapy with overall treatment plans.
- Maintain accurate and comprehensive patient treatment records to support ongoing care and accountability.
Requirements:
- HCPC registration is a must.
- Minimum of 2 years' experience in unplanned care physiotherapy.
- Car driver preferred for efficient travel between sites.
