What you will do:

- Assess and evaluate patients' condition to create tailored rehabilitation plans for stroke recovery.
- Implement evidence-based therapy interventions to improve patients' functional abilities and autonomy.
- Collaborate with a multidisciplinary team to ensure the highest quality of patient care and optimal recovery outcomes.
- Monitor patient progress and adjust treatment plans as necessary to achieve therapy goals.
- Engage with patients and families to provide education and support throughout the rehabilitation process.

Requirements:

- Degree in Occupational Therapy with a post-graduate qualification preferred but not essential.
- Enhanced community-based stroke experience.
- Proficiency with SystmOne is desirable, however not essential.
- HCPC registration is required.
- Must be a car driver with access to a vehicle, and comfortable with covering Hull and East Riding areas.
