What you will do

• Rehabilitation Support: Deliver high-quality occupational therapy interventions for patients requiring complex medical rehabilitation.
• Discharge Planning: Lead effective discharge planning to ensure smooth transitions from hospital to home or community settings.
• Case Management: Manage your own caseload independently, prioritising patient needs and adapting to changing clinical demands.
• Multidisciplinary Working: Collaborate with medical and allied health professionals across various wards to provide holistic care.
• Treatment Planning: Adapt and review therapy plans to ensure they are responsive to patient progress and evolving needs.

Qualifications / Requirements

• HCPC registered Occupational Therapist.
• Experience within acute or medical rehabilitation settings.
• Strong discharge planning and case management skills.
• Ability to work autonomously and under pressure within a multidisciplinary team.
