What you will do:
- Assess and plan patient discharge in collaboration with a multi-disciplinary team
- Develop personalised occupational therapy programmes to facilitate efficient patient discharge

- Monitor and evaluate patient progress and modify plans accordingly to support successful discharge
- Maintain accurate and up-to-date patient records and reports
Requirements:
- HCPC registration is essential
