Job Title: Band 6 Community Psychiatric Nurse – Early Intervention Psychosis

What you will do:

• Deliver high-quality care and treatment to individuals experiencing the early stages of psychosis in a community setting.

• Work collaboratively with the multidisciplinary STEP team to provide evidence-based interventions tailored to each service user's needs.

• Engage with service users and their families to create care plans that promote recovery and social inclusion.

• Monitor and assess patient progress, adapting care plans as necessary to ensure the best possible outcomes.

• Liaise with external agencies and services to coordinate care and support a seamless pathway for service users.
