What you will do:

  • Undertake a comprehensive assessment of service users who meet the service eligibility criteria, working in partnership with their family to tailor supportive care plans.
  • Design, implement, and review individual care plans and risk assessments, utilizing evidence-based interventions to maximize benefit for service users.
  • Respond efficiently to acute clinical presentations, taking a proactive approach to manage risk and ensure the safety and well-being of service users.
  • Apply clinical judgment to assess and respond to service users’ physical and psychological needs, adapting treatment plans accordingly.
  • Implement thorough risk management systems to elevate clinical safety and service user satisfaction within your area.
  • Ensure a swift and coherent response to safeguarding incidents, adhering to departmental guidelines.
  • Deliver an effective response to individuals experiencing crises, coordinating timely interventions to stabilise situations.
  • Lead day-to-day management duties, including team support and resource allocation, under the guidance of the Team Leader.
  • Collaborate with colleagues and partners effectively to enhance service delivery and fulfil organisational goals.
  • Plan and deliver treatment interventions aligned with Best Practice standards and relevant guidelines.
  • Operate as a Care Coordinator, ensuring exemplary care service at all times.
  • Champion service development or improvement projects as designated.
  • Support management in the timely resolution of complaints and significant incidents.
  • Identify and communicate the training needs of junior staff to the Team Leader; plan and deliver necessary training sessions.
  • Undertake or be willing to pursue training as an Approved Mental Health Professional (AMHP).
