What you will do:

• Acute Assessments: Complete comprehensive physiotherapy assessments within an acute hospital setting.

• Treatment Planning: Develop and implement tailored treatment programmes to meet individual patient needs.

• Multidisciplinary Working: Collaborate closely with consultants, nurses, occupational therapists, and other allied health professionals.

• Caseload Management: Prioritise and manage a busy caseload effectively based on clinical urgency.

• Discharge Planning: Support safe and timely discharges, including onward referrals to community services.

• Clinical Documentation: Maintain accurate, clear, and up-to-date patient records.

• Patient Education: Provide advice and education to patients and families to support recovery and independence.

• Continuous Development: Engage in CPD to stay aligned with best practice and current evidence.

Responsibilities and general duties:

• Experience: Previous Band 6 experience with recent work in acute medical settings, including ageing health and frailty.

• Discharge Expertise: Strong skills in discharge planning and patient flow management.

• Registration: HCPC registration is essential.

• Minimum Experience: At least 2 years’ relevant post-qualification experience.

• Driving: Being a car driver is preferable to support travel across sites if required.
